Lifepoint: A Comprehensive Transition Service

LifePoint is a comprehensive transition program serving Medicaid enrolled young adults ages 17-21, in need of assistance developing and/or implementing independent living strategies. The program provides comprehensive services to help bridge the gap from adolescence to adulthood.

Residing in a safe, structured and supervised environment, individuals learn and practice skills to prepare them for a fully engaged community life. The program aims to meet identified goals within a 6-month time frame by uniquely tailoring services to the individual.


  • Medicaid eligible
  • At this time, LifePoint serves Males Only (17-21)
  • Willing to participate in a residential program
  • Able to safely participate in community activities i.e. employment, school, bus, recreation.
